How do I use Recare Predict?

How Recare Predict helps you identify and manage early on which patients are likely to need a transfer to aftercare.

Note: Recare Predict calculates a transfer need (the probability that aftercare or a transfer will be required) for each inpatient, helping discharge management identify, prioritise and document cases early. Predict is a clinic module and must be enabled in your contract.

Opening Predict

  1. Log in to Recare.

  2. At the top left next to the Recare logo, use the dropdown to choose the Predict view instead of "Dashboard".

  3. At the top right, select the clinic (via the dropdown if you have several sites). The top right also gives you access to your account, customer service, the newsletter and the FAQ.


The patient list

You see a list of the relevant patients with ward, age, diagnosis (ICD-10), Barthel Index, admission date, length of stay and the transfer need as a percentage.

  • Sort: e.g. by length of stay (click the column).

  • Search by case number: use the magnifying glass to enter the hospital case number and jump straight to a patient.


Editing a case

Open a patient or scroll right in the row:

  • Choose the status:

    • Offen (open) – not yet decided; stays in the pool of cases to handle.

    • Überleitung nicht nötig (no transfer needed) – no aftercare needed; the patient can go home without support.

    • Überleitung gestartet (transfer started) – a search is already running in Recare (Discharge); nothing further needs to be triggered from Predict.

    • Gemeldet (flagged) – someone is responsible or the case has been discussed, but a final decision is still pending.

  • Assign a responsible employee (e.g. yourself, or a colleague from the list when taking over a case).

  • Rating (thumbs up/down): rate whether the prediction fits this case – this helps review and train the model (e.g. thumbs down for a high need when clearly no aftercare is required).

  • Bookmark: flag a patient to find them later via the marked patients filter.

  • If the note "refresh to see changes" appears, click it to update the view.


Details and explanation of the calculation

Via Details you open the individual view. Under explanation of the calculation you see how the percentage is composed – e.g. how much age, primary and secondary diagnosis, gender, Barthel Index and care level contribute. You also find patient data such as date of birth, gender, care level, address, health insurer, insurance number, primary and secondary diagnoses and the admission date.


Filtering the list

Use the blue Filter button to narrow the list (multiple selection possible):

  • Ward (e.g. cardiology, pulmonology)

  • Specialist department (e.g. internal medicine)

  • Status (open, no transfer needed, transfer started, flagged)

  • Responsible employee

  • Transfer need (low, medium or high probability)

  • Admission date (via the calendar icon)

  • Marked patients (your bookmarks)

Click Filter anwenden (apply filters). Active filters appear as chips and can be removed individually.

Tip: At the start of your shift, bookmark the patients you want to handle today and open them together via the marked patients filter.


FAQs

What does the transfer need percentage mean?

It is the model's calculated probability that a transfer or aftercare will be needed for this patient. Via Details → explanation of the calculation you can see which factors contribute and how strongly.

What do the different statuses mean?

"Open" = still to be decided; "no transfer needed" = no aftercare; "transfer started" = a search is already running in Recare; "flagged" = responsible/discussed, but still open.

What is the thumbs up/down for?

To give feedback on whether the prediction fits. This helps review and further train the Predict model.

How do I find my bookmarked patients again?

Via the marked patients filter – it shows all cases you have bookmarked.
